Obligations of Calm and Connected:
- Allow the Customer to take and use the Equipment for the Loan Period;
- Provide the Equipment to the Client clean and in good working order;
- Collect the Equipment at the next scheduled session or within the specified period written on the loan form or within 5 days of being requested to do so by the Client
- Provide a demonstration and explanation on how to use the equipment safely
- Provide in writing any precautions that may apply to the piece of equipment
- Be available by phone or email to answer questions on the equipment loaned
Obligations of the Client:
-Return the equipment at the next scheduled session or within the specified period written on the loan form or contact Calm and Connected when they wish to have it returned
- All equipment must be returned in its entirety, and in the same condition and working order as when borrowed
- The borrower is liable for any damages or loss of loan items
- Use the equipment in accordance to how it was demonstrated by therapist
- Contact therapist if have any questions or concerns
- Report any damage or loss to therapist within 2 business days of the damage or loss occurring
- Pay for any loss or damage made to the Equipment that was loaned
Payment for damages or replacement
Clients are required to sign up to our secure direct debit system as security for payment for any damage or replacement needed for items. Calm and Connected will inform you of any costs prior to taking payment.
